By 1989, treatment center referrals accounted for 40% of new AA memberships (Mäkelä et al., 1996). This standard persisted in SUD treatment even as strong evidence emerged that a minority of individuals who receive 12-Step treatment achieve and maintain long-term abstinence (e.g., Project MATCH Research Group, 1998). The revised dynamic model of relapse also takes into account the timing and interrelatedness of risk factors, as well as provides for https://ecosoberhouse.com/ feedback between lower- and higher-level components of the model. For example, based on the dynamic model it is hypothesized that changes in one risk factor (e.g. negative affect) influences changes in drinking behavior and that changes in drinking also influences changes in the risk factors. The dynamic model of relapse has generated enthusiasm among researchers and clinicians who have observed these processes in their data and their clients.

Such a framework should not only include predictors that are known from prior models, such as Marlatt’s Relapse Prevention Model, but also predictors that have been newly identified in this study and other recent studies (Kwasnicka, Dombrowski, White, & Sniehotta, 2019; Roordink et al., 2021). For example, in this study self-value and resilience received high importance ratings, but these are not reflected in current models. We believe a theoretical framework based on the latest insights would be of added value to the field of relapse prevention and can inform future weight loss maintenance interventions. Second, for several predictors scientific evidence for a direct association with relapse in weight loss maintenance behaviors is lacking in prior research. Therefore, to examine whether the identified perceived predictors in this study indeed predict relapse in weight loss maintenance behaviors, a larger prospective study is recommended. We suggest an ecological momentary assessment (EMA) study to track experiences over time and get insight into the process of behavior change, among which lapsing and relapsing (Shiffman, Stone, & Hufford, 2008).

Models of nonabstinence psychosocial treatment for SUD

There is also a need for updated research examining standards of practice in community SUD treatment, including acceptance of non-abstinence goals and facility policies such as administrative discharge. Individuals with fewer years of addiction and lower severity SUDs generally have the highest likelihood of achieving moderate, low-consequence substance use after treatment (Öjehagen & Berglund, 1989; Witkiewitz, 2008). Notably, these individuals are also most likely to endorse nonabstinence goals (Berglund et al., 2019; Dunn & Strain, 2013; Lozano et al., 2006; Lozano et al., 2015; Mowbray et al., 2013). In contrast, individuals with greater SUD severity, who are more likely to have abstinence goals, generally have the best outcomes when working toward abstinence (Witkiewitz, 2008).

When abstinence is violated, individuals typically also have an emotional response consisting of guilt, shame, hopelessness, loss of control, and/or a sense of failure; they may use drugs or alcohol in an attempt to cope with the negative feelings that resulted from their abstinence violation. A person may experience a particularly stressful emotional event in their lives and may turn to alcohol and/or drugs to cope with these negative emotions. An abstinence violation can also occur in individuals with low self-efficacy, since they do not feel very confident in their ability to carry out their goal of abstinence.

The reformulated cognitive-behavioral model of relapse

She is a current member of the Golden Key International Honor Society and the Delta Epsilon Iota Honor Society. If you have completed a drug or alcohol treatment program, then you are probably considering trying to rebuild your life. The weight of this guilt often correlates abstinence violation effect to the amount of time spent in recovery leading up to the relapse. Those with only a few weeks of sobriety will not feel as bad as those with years under their belt. Not out of the same warped practicality mentioned above, but because they simply feel as if they are hopeless.
